Mountainlands Family Health Center (MFHC) is a high-quality, patient-centered medical home for everyone, including the uninsured and most vulnerable individuals within our community. Our mission is to provide affordable, comprehensive health care services including primary medical, dental, mental health, and pharmacy services to ALL individuals within our community, regardless of their ability to pay. Mountainlands is the only comprehensive primary care provider in Utah and Uintah Counties utilizing a sliding fee scale based on income and family size, and the only free homeless health clinic in Utah County.

Our organization is passionate about the health of all of our community members –especially the youth in our area. We are proud to support The Utah Valley Drug Prevention Coalition in their effort to reduce the misuse of marijuana, alcohol, and prescription drugs among the young people in our area.

Mountainlands Family Health Center provides primary health care services, which include:

Our Outreach & Enrollment counselors are available to assist patients with health insurance, Medicare, Medicaid, and CHIP enrollment. All MFHC patients are eligible for prescription medications at a discounted price through our on-site pharmacies or participating Smith’s Pharmacies.

We currently have six health center locations. Our main site is located at 589 South State Street in Provo. Other centers are located in Vernal, Payson, and co-located within Wasatch Mental Health in Provo (see business hours at www.mountainlands.org).

The East Bay Health Center is co-located with the Food and Care Coalition in Provo. This clinic is specifically designed to serve individuals experiencing homelessness by providing them free access to medical, dental, pharmacy and behavioral healthcare services. It is the only health clinic for homeless individuals in Utah County.
